Process to Screening Entry/Update
- Click on the Action button for required data >> Screening Entry/Update
- Fill up the WGQ questionnaire that consists of various domains of disabilities.
- Choose required radio button to fill up the questionnaire. User may choose multiple domains of disabilities for screening entry. The screening entry form is based on the WGQ method and uses Skip logic. Screening Questionnaire will be updated by school, health post, or health camp.
- Click on Save button for completion. A Referral Card will be generated based on the data entered in the WGQ questionnaire form.
- Select the Class and click Load for data population.
